Immigration Medical Services

INcircle Health offers immigration medical examinations required by U.S. Citizenship and Immigration Services (USCIS). Exams are conducted by Dr. Juan Carlos Venis, a USCIS-designated civil surgeon and board-certified family physician, in a respectful, private, and welcoming clinical setting.

Our Services

  • We provide U.S. Citizenship and Immigration Services (USCIS)–required immigration medical examinations for Form I-693, which is part of the adjustment of status process for individuals applying for lawful permanent residency (a green card). A designated civil surgeon conducts a complete medical history review, physical examination, required laboratory testing, and vaccination assessment in accordance with current USCIS guidelines.

    This visit is limited in scope and focused solely on meeting USCIS requirements. The completed I-693 form is prepared accurately and sealed for submission to USCIS. Immigration medical exams are offered as a self-pay service with transparent pricing and are separate from primary care membership services.

  • We offer comprehensive medical evaluations for the U.S. Citizenship and Immigration Services (USCIS) Form N-648, which allow applicants for U.S. citizenship to request an exception to the English and/or civics testing requirement due to a qualifying medical condition. These evaluations are conducted in accordance with USCIS requirements and include a detailed medical history review, cognitive or functional assessment when appropriate, and completion of the N-648 form if criteria are met.

    This service is provided independently from primary care membership and is offered for a transparent, self-pay fee. We take time to ensure the evaluation is thorough, accurate, and clearly documented to support your application with USCIS.

  • The total fee includes:

    • $50 non-refundable scheduling deposit

    • Review of immigration paperwork and medical history

    • USCIS-required physical examination

    • Vaccination record review and determination of required vaccines based on USCIS guidelines

    • Ordering and coordination of all required laboratory testing associated with the I-693 exam

    • Completion of USCIS Form I-693 by a designated civil surgeon

    • Sealing of the completed I-693 form for USCIS submission

    • Clear instructions on next steps and document handling

    Vaccinations:
    Vaccines are not administered on site. If additional vaccines are required, patients are referred to trusted local options that offer free or low-cost vaccines. Patients may choose the most convenient location and are responsible for vaccine-related costs.

    Not included:

    • Vaccines or vaccine administration

    • Care unrelated to the immigration exam

    • Follow-up medical treatment

    • Imaging studies (such as chest X-rays), which are referred out if required and paid separately

  • The total fee includes:

    • $50 non-refundable scheduling deposit

    • Comprehensive review of medical, psychiatric, and functional history

    • Evaluation of the condition(s) impacting the ability to meet English and/or civics testing requirements

    • Cognitive or functional assessment when clinically appropriate

    • Review of supporting medical records (when available)

    • Determination of whether USCIS criteria for the N-648 are met

    • Accurate completion of USCIS Form N-648 if eligibility criteria are satisfied

    • Time spent ensuring documentation is clear, thorough, and compliant

    Important notes:

    • Completion of the N-648 is based on medical eligibility and is not guaranteed

    • This evaluation is separate from primary care and does not establish an ongoing doctor-patient relationship

  • USCIS requires certified interpretation services for immigration medical examinations when the applicant is not fluent in English. When interpretation is required, a qualified interpreter must be present for the exam.

    • Certified interpretation services can be provided by the clinic for an additional $100 fee

    • Patients are able to provide their own certified interpreter provided there is no conflict of interest

    • The interpreter must be able to provide appropriate credentials

    • The interpreter must sign the required sections of the USCIS form when applicable

    • Family members or friends may not serve as interpreters at our clinic

    If you prefer to bring your own interpreter, please notify us in advance to ensure they meet documentation and credentialing requirements.

  • I-693 Immigration Medical Examination – Important Information

    The I-693 Immigration Medical Examination is a limited-scope visit required by U.S. Citizenship and Immigration Services (USCIS) as part of the adjustment of status process. This visit is not a comprehensive primary care appointment and does not establish an ongoing doctor-patient relationship.

    The total fee for the I-693 exam is $375, which includes the medical evaluation, completion of USCIS Form I-693, and required in-office laboratory testing. Insurance is not billed for this service. A $50 non-refundable deposit is required at the time of booking to reserve your appointment. The remaining $325 balance is due at check-in on the day of your visit. Accepted payment methods include cash, check, ACH, or credit/debit card (processing fees may apply).

    Vaccines are not administered on site. Vaccination records will be reviewed to determine compliance with USCIS requirements. If additional vaccines are required, patients will be referred to local resources such as retail pharmacies, the health department, or community immunization clinics. Vaccine costs are paid separately but in some cases can be obtained at no cost.

    Additional services may be required based on USCIS guidelines or exam findings and are not included in the $375 fee. These may include, but are not limited to:

    • Chest X-rays or other imaging

    • Outside referral for evaluation or treatment of communicable disease (eg. TB)

    • Vaccinations

    • Certified interpretation services

    • Outside laboratory studies

    Any additional requirements will be discussed with you in advance and paid separately.

    If you are not fluent in English, USCIS requires certified interpretation services. Interpretation can be provided by the clinic for an additional $100 fee. Interpreters must provide appropriate credentials and may be required to sign USCIS documentation. If you bring your own interpreter, they must meet USCIS requirements and be approved in advance.

    Please bring all available vaccination and medical records to your appointment. If records are incomplete or unavailable, additional testing or vaccines may be required. Completion of the I-693 exam does not guarantee USCIS approval. Our role is limited to performing the required medical evaluation and completing documentation in accordance with USCIS standards.

  • N-648 Medical Disability Exception Evaluation – Important Information

    The N-648 Medical Disability Exception Evaluation is a limited-scope medical evaluation conducted in accordance with requirements set by U.S. Citizenship and Immigration Services (USCIS). The purpose of this visit is to assess whether a qualifying medical, cognitive, or psychiatric condition prevents an applicant from meeting the English and/or civics testing requirements for U.S. naturalization. This visit is not a comprehensive primary care appointment and does not establish an ongoing doctor-patient relationship.

    The total fee for the N-648 evaluation is $450. This is a self-pay service, and insurance is not billed. A $50 non-refundable deposit is required at the time of booking to reserve your appointment. The remaining $400 balance is due at check-in on the day of your visit. The fee includes a detailed review of medical and functional history, clinical assessment as appropriate, review of supporting medical records when available, and completion of USCIS Form N-648 if medical criteria are met. Accepted payment methods include cash, check, ACH, or credit/debit card (processing fees may apply).

    Completion of Form N-648 is not guaranteed. USCIS approval depends on whether the applicant meets strict medical eligibility criteria and on USCIS’s independent review of the application. Our role is limited to performing an objective medical evaluation and accurately documenting findings in accordance with USCIS requirements.

    If you are not fluent in English, USCIS requires certified interpretation services for the evaluation. Interpretation can be provided by the clinic for an additional $100 fee. Interpreters must provide appropriate credentials and may be required to sign USCIS documentation. If you bring your own interpreter, they must meet USCIS requirements and be approved in advance.

    Please bring all relevant medical records, prior evaluations, and supporting documentation to your appointment. Incomplete or unavailable records may limit the ability to complete the evaluation or may require additional visits, which are not included in the initial fee.

* I-693 Patients: If you have any vaccination records, please bring them to your visit. Read below for more information regarding immunizations.

Follow-up as applicable and receive completed paperwork.

Depending on individual case, patients will be responsible for obtaining vaccines or further testing. Complete evaluation depends on result of initial visit, immunization review, and lab work. Appropriate follow-up will be arranged to complete process and provide patient with completed paperwork. If outside records are required, the paperwork can be completed when this is obtained. In some cases, the paperwork can be completed the same day.

Overview

U.S. immigration law requires applicants for adjustment of status to meet specific age-appropriate vaccination requirements as determined by the CDC and U.S. Citizenship and Immigration Services.

To complete Form I-693 at INcircle Health, applicants must meet one of the following for each required vaccine:

  • Provide acceptable documentation of prior vaccination,

  • Provide laboratory evidence of immunity (positive titers) when appropriate.

Vaccines are not administered at INcircle Health but can be received at any of the recommended outside locations, including those that offer free or low-cost vaccines. Other clinics offering immigration exams may charge for vaccines, so we aim to save you money.

Varicella (Chickenpox)

If you do not have documentation of prior varicella vaccination but believe you had chickenpox in the past, immunity needs to be confirmed.

If immunity cannot be confirmed by history alone, varicella antibody testing may be required. Patients may also opt to get the immunization.

Immunization Requirements for Form I—693

Choosing Immunity Testing Instead of Vaccination

For patients who wish to avoid receiving vaccines, laboratory testing for immunity may be an option for the following vaccines:

  • Measles

  • Mumps

  • Rubella

  • Hepatitis A

  • Hepatitis B

  • Varicella

Any immunity testing beyond varicella is not included in the exam fee and will be discussed with you in advance if needed. Outside lab fees are paid separately.

Please bring all available vaccination and medical records to your appointment to help avoid unnecessary vaccines or testing and ensure timely completion of your I-693.

While we understand that patients are anxious to get their vaccination requirements completed, we ask that you wait for your appointment to review this with the doctor. If you have paid your deposit, scheduled your visit, and you have vaccine records in your possession, you may choose to send them to our office by email at hello@incirclehealth.com. We cannot guarantee review and recommendations prior to the appointment.
